Some people (mainly women) develop valgus heel and subsequent flatfoot deformity in their adult life.
The patient typically complains of pain located at the posterior aspect of the foot along with noted progressive valgus deformation of the heel. This situation is caused by tibialis posterior tendonitis which is progressively attenuated and worn. Walking becomes a painful activity.
In some cases, this clinical situation is noted in adolescence, and it is caused by defective separation of some bones during growth.
Various foot supports are commercially available, yet their clinical significance is very limited. Surgical treatment can significantly improve function and includes combinations of tendon transfers, osteotomies, and arthrodesis.
Surgery is performed under regional or mild general anesthesia. The patient can be discharged the same day with plaster cast or splint for some weeks depending on the type of surgery. Weight bearing is usually allowed after 4 weeks
